When your records are ready, we’ll send you a password-protected link to download your files. No one else will have access. As for the original microfiche, you choose what happens next. If you’d like it returned, just include a stamped envelope when you send it in. If you’d rather we destroy it, we’ll securely shred it after 10 days. Either way, your information stays private and protected.

How Does This Service Work

We’ve kept the process as straightforward as possible to try and alleviate any issues or stress on your end. Here is all you need to do:

  1. Print and fill out this form with your name, email, and phone number. Check the box to tell us whether you want your original microfiche returned or securely shredded. If you’d like it returned, include a stamped return envelope with your address.
  2. Place the completed form and your microfiche in an envelope (along with the stamped return envelope, if desired) and mail it to:
